研究目的は,A大学4年次精神看護学実習で,病棟実習をおこなった3グループと,社会復帰施設実習をおこなった3グループの学生が考えた,精神看護の意味や役割に焦点をあて,学びの特徴を明らかにすることである.分析対象は,6グループ(病棟3グループ,施設3グループ)46名が実習後に提出したレポートの内容である.分析方法は,学生が考えた精神看護の意味や役割に関する記述を実習施設別に抽出し,内容の類似するものをまとめてサブカテゴリとし,カテゴリ,コアカテゴリ化した.記述の抽出,カテゴリ化では,研究者間で繰り返し検討し,信用性の確保に努めた.両者の実習施設に共通するものとそれぞれの実習施設の特徴を分析した.受け持ち患者の特徴は,病棟はうつ病が最も多く,成人期と老年期の回復期の患者が大部分であった.社会復帰施設は統合失調症が多く,成人期の長期在宅生活者が大部分であった.学びを抽出した結果,135サブカテゴリ,45カテゴリ,11コアカテゴリに分類した.各施設において多くの共通した精神看護の意味・役割を学んでいた.一方で,各施設において患者層の違いや実習場所の特徴により学びに相違がみられた.病院実習では急性期の症状に対する看護や退院に向けた短期的視点での看護を考えることが多く,看護師が実践モデルを示すために精神看護の意味や役割について学生が理解しやすかった.また社会復帰施設では,慢性期の長期在宅精神障害者に対して疾患をコントロールしながら地域生活を送るための支援という中・長期的視点での看護を考えることが多く,指導員や精神保健福祉士の対象への関わりや教員が実践モデルを示す中で,学生が看護として何が出来るかを日々考える実習を行っていた.これ

The purpose of this study is to compare three groups which received practical training at a rehabilitation institution with three groups which received practical training in a psychiatric ward, and to clarify the characteristics regarding how the students learned about the significance and the roles of psychiatric nursing at each training institution. The findings are based on 46 reports which were submitted by the students after the training. In the ward, the patients mostly consisted of cases of depression or dementia. At the rehabilitation institution, most patients had schizophrenia; and they had also lived at their homes for a long period of time. According to the results of the analysis, the students learned about the significance of psychiatric nursing and the roles that such nurses played in the hospitals. The various learning (factors) were then classified into 135 sub categories, 45 categories, and 11 core categories. The students learned a lot about the significance of psychiatric nursing and the roles that such nurses (or psychiatric social workers) played which were common to both institutions. On the other hand, due to differences in the patients and the unique characteristics of each training facility, the students therefore learned different things at each facility. These differences were as follows; In ward training, the students thought about nursing in regard to caring for acute stage symptoms of such patients after they are discharged from the hospital. At the rehabilitation institution, the students thought about nursing in regard to supporting the patients' daily life while they help such patients to also control their chronic mental disorders. It is therefore considered to be important that the students learn the different viewpoints of the nurses and social workers working at both institutions. As a result, more appropriate training methods should therefore be established in the future.
